Yoorana Gunya covers the ITC Program for Region 5: Forbes, Parkes, Cowra and Weddin.

The ITC Program aims to achieve better outcomes for Aboriginal and Torres Strait Islander people by assisting patients diagnosed with a chronic condition. Our ITC team works together with patients, health professionals and specialists for patient’s appointments and health outcomes.

The ITC Program can assist patients accessing supplementary services such as:

• Assistance or transport attending specialist appointments

• Support with purchasing medical aids as suggested by a specialist
  

To be eligible for the ITC program, Aboriginal and Torres Strait Islander people of any age should have one or more of the following chronic diseases or conditions:

• Brain diseases

• Lung diseases

• Kidney diseases

• Heart diseases

• Cancer

Referral information

Patients must Have a diagnosed chronic disease and GP Management Plan and referral from a General Practitioner. All referrals are to provide a minimum of 48 hours’ notice for processing. Please be aware that due to a limited funding allocation, not all referrals will be accepted. Referrals will be prioritised and accepted as per our triage system.
